ATTENTION 7th Grade
Get your shots before school starts and turn in your immunization records!
Updated physical examinations and immunizations are required for Palm Beach County students entering 7th grade and must be presented on forms DH 3040 and DH 680. Please see your health care provider for the forms. Completed forms should be turned in to the school front office (over the summer, they can be dropped off Mon-Thurs). 7th graders will not be permitted to pickup class schedules without the original, completed forms on file.
